Pulse Care Network is committed to transparency in how we operate, how we are compensated, and how the programs we represent are structured. The following disclosures are provided in accordance with applicable federal and state requirements and as a matter of professional practice.

1. Independent Agent Status

Pulse Care Network is an independent benefits consulting practice operated by Mark Scheribel. We are not an insurance carrier, plan sponsor, plan administrator, healthcare provider, or employer. Pulse Care Network facilitates employer access to workforce health screening and supplemental benefits programs. We do not sell, solicit, or negotiate insurance products.

2. Programs We Represent

PulseWorx Cardiovascular Screening

PulseWorx cardiovascular screening services are provided by Pulse4Pulse, based in Middlebury, CT. Pulse Care Network serves as an independent agent that schedules and facilitates employer access to PulseWorx screening events. All screening services are delivered by Pulse4Pulse's medical team. Pulse Care Network does not employ healthcare providers and does not deliver clinical services.

Pulse 365 Wellness Supplemental Benefits

Supplemental clinical benefits described on this Site are provided by Pulse 365 Wellness (pulse365wellness.com). These benefits are delivered through an IRC Section 125 Cafeteria Plan structure. Pulse Care Network serves as an independent agent connecting employers with Pulse 365 Wellness and is not the plan sponsor, plan administrator, or insurer.

3. Compensation Disclosure

In accordance with Section 202 of the Consolidated Appropriations Act of 2021 (CAA 2021) and ERISA Section 408(b)(2), Pulse Care Network hereby discloses the following regarding our compensation arrangements:

PulseWorx / Pulse4Pulse

Pulse Care Network receives a one-time fee per employer engagement from Pulse4Pulse upon successful scheduling and delivery of a PulseWorx screening event. The amount of this fee varies by engagement and will be disclosed in writing prior to any contract being entered into.

Pulse 365 Wellness

Pulse Care Network receives a per-employee-per-month (PEPM) commission from Pulse 365 Wellness for employer accounts we place. This is indirect compensation funded by the program structure, not charged additionally to the employer. The specific PEPM amount will be disclosed in writing prior to any contract being entered into, extended, or renewed.

We will provide updated compensation disclosures within 60 days of any material change to our compensation arrangements, in accordance with CAA requirements. Employers may request a written compensation disclosure at any time by contacting us at the address below.

4. Not a Fiduciary

Pulse Care Network does not act as a fiduciary to any employer health plan. Employers who implement benefits programs through our referrals retain their own fiduciary responsibilities under ERISA as plan sponsors. We strongly recommend that employers consult with independent ERISA counsel regarding their fiduciary obligations.

5. Not Medical, Legal, or Tax Advice

Nothing on this Site or in any communication from Pulse Care Network constitutes medical, legal, tax, or financial advice. Clinical statistics and health information presented are for educational purposes only. Employers should consult qualified medical, legal, and tax professionals before implementing any wellness or benefits program.

The Section 125 Cafeteria Plan structure referenced in connection with Pulse 365 Wellness has been reviewed by ERISA counsel at the program level. However, each employer's specific tax situation is unique. Pulse Care Network does not provide tax advice and cannot guarantee any specific tax outcome for any employer.

6. HIPAA

Pulse Care Network does not collect, access, store, or process protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act (HIPAA). Individual employee health screening data from PulseWorx events is handled exclusively by Pulse4Pulse under their own HIPAA-compliant framework. Supplemental benefits data under Pulse 365 Wellness is handled by Pulse 365 Wellness and their plan administrator under their respective HIPAA frameworks.

7. Licensing

Pulse Care Network facilitates employer access to workforce health screening and supplemental benefits programs. We do not sell, solicit, or negotiate insurance products.
